ABSTRACT

Purpose. To demonstrate possibilities of multispiral computed tomography in the detection of fungal osteomyelitis in two patients with second type of diabetes mellitus two and six months after COVID-19 associated pneumonia. Material and methods. We present two clinical observations of patients with second type of diabetes mellitus who complained about pain in the upper jaw, nasal purulent discharge, difficulty in nasal breathing which appeared two and six months after COVID-19 associated pneumonia. To identify pathological changes, patients were directed to the department of radiology for the purpose of performing MSCT of the skull. Results. In the presented clinical cases, using MSCT, the features of the radiological semiotics of skull bone changes were studied in patients with fungal infection on the background of type 2 diabetes mellitus, the use of corticosteroids and after the viral COVID-19 associated pneumonia. The diagnosis was verified by histological and cytological studies of biopsy (surgical) material. Discussion. Recently, there has been an increase in the number of cases of fungal osteomyelitis of the jaws and paranasal sinuses in patients after viral pneumonia caused by SARS-CoV-2. The most common fungal infection is mucormycosis, caused by fungi belonging to the order Mucorales. The rhinocerebral form of mucormycosis is common in patients with diabetes mellitus after treatment with corticosteroids with the background of immunosuppression. This form of mucormycosis contributes to the appearance of extensive bone-destructive changes in the middle and upper zones of the maxillofacial region, requiring further surgical treatment. Conclusion. The 2019 coronavirus infection (COVID-19) caused by SARS-CoV-2 and type 2 diabetes remain urgent healthcare problems worldwide. This combination in a patient after treatment of COVID-19 associated pneumonia with corticosteroids leads to immunosuppression and the development of concomitant infections, including fungal ones. Fungal osteomyelitis in such patients, as a rule, affects the skull and is characterized by an aggressive course and requires surgical treatment. The use of modern and high-tech methods of radiation imaging, such as MSCT, allows to obtain complete diagnostic information about the localization and prevalence of the lesion, which, in the future, determines the management tactics and surgical treatment of patients of this category.Copyright © 2022 Chinese Journal of Pediatric Surgery. ABSTRACT

Purpose: In many healthcare settings in developing nations, multislice computed tomography (MSCT) imaging may be the only available diagnostic modality for patients with suspected COVID-19 infection, due to a shortage of laboratory kits. This study aimed to evaluate the diagnostic performance and interobserver variability of CO-RADS (COVID-19 Reporting and Data System) in the triage of patients with suspected COVID-19 infection in Zagazig University Hospital. Material and methods: This study included 2500 patients with suspected COVID-19 infection, mean age 60.61 years ± 13.89. 61.4% were male. Unstable patients requiring urgent invasive ventilation, acute coronary syndrome patients, pregnant females, and patients with RT-PCR results available prior to MSCT were excluded from this study. RT-PCR was performed in all patients included in the study. Results: Fever and dry cough were the most common clinical symptoms, detected in 80.16% and 52.00%, respectively. The most common comorbidities were cardiovascular diseases, followed by chronic lung disease and diabetes, found in 27.36%, 22.80%, and 18.00%, respectively. Of the 1500 RT-PCR-positive patients, 40% had CO-RADS score 5, while 3.4% had CO-RADS score 1. Of the 1000 RT-PCR-negative patients, 36% had CO-RADS score 2 and 1% were scored as CO-RADS 5. There was excellent agreement in the studied patients as the weighted κ value was 0.846, which was more pronounced at CO-RADS 5 (24.40%). The sensitivity of CO-RADS was higher in the 2nd scenario (83.27% vs. 55.27%) while the specificity was higher in the 1st scenario (95% vs. 65%). Conclusion: The CO-RADS scoring system is a sensitive and specific method that can help in the diagnosis of COVID-19 during the peak of the COVID-19 pandemic. CO-RADS is a triage test in resource-constrained environments, assisting in the optimization of RT-PCR tests, isolation beds, and intensive care units.

ABSTRACT

Aneurysms represent bulging of the weakened blood vessel area, as a result of cystic medial degeneration. Aneurysms chance of rupturing increases over time, resulting in bleeding and death. Therefore, patients with aortic aneurysms require frequent monitoring with magnetic resonance and computed tomography angiography, as well as undergoing open repair surgery and endovascular aneurysm repair. We present a case of ruptured thoracic aortic aneurysm in conjunction with bilateral superficial femoral occlusion, as incidental findings in Covid-19 positive patient. The patient, a 62-years-old female, presented with cough, shortness of breath, fever and leg claudication. Doppler ultrasonography of the lower limbs was conducted to rule out thromboembolism, revealing bilateral superficial femoral arteries occlusion. The patient was administered high doses of parenteral anticoagulants. Hemoptysis ensued, prompting an MSCT scan, that showed right pleural effusion, atelectasis, and right active perihilar infiltrates with inter-lobar pleurisy. Due to inflammatory changes on the lung parenchyma, the patient got tested for Sars-Cov-2, and resulted positive. Contrast-enhanced MSCT also revealed thoracic-abdominal aortic aneurysm with its highest diameter measuring 10 cm, and massive per-aortal thrombus and/or hematoma of 5 cm, which was further ruptured and patient died untreated in the fourth day of hospitalization. Questions arise whether Covid-19 was the primary cause of bilateral superficial femoral artery occlusion and whether high doses of parenteral anticoagulants were the primary cause of thoracic aortic aneurysm rupture. Thus, a careful balance must be made between the detrimental and protective contributions of anticoagulants in the patients presenting with Covid-19 and thoracic-abdominal aortic aneurysm.
